Understanding Intrauterine Insemination (IUI) After Tubal Reversal

For many women who have undergone tubal ligation, the decision to reverse the procedure often comes with the hope of expanding their families. Tubal reversal surgery, also known as tubal reanastomosis, reconnects the fallopian tubes, allowing eggs to travel from the ovaries to the uterus once again. However, achieving pregnancy post-reversal can sometimes require additional fertility treatments, such as tubal testing/ flushing (HyCoSy), ovulation induction (e.g. Clomid/ Letrozole) or intrauterine insemination (IUI). It is more likely you will require one of the above options if you do not conceive within 6-12 months following your surgery. As such please get in touch if you haven’t conceived after 6 months.
What is IUI?
Intrauterine insemination (IUI) is a fertility treatment that involves placing sperm directly into a woman’s uterus around the time of ovulation. This procedure increases the number of sperm that reach the fallopian tubes (by millions of sperm!), thereby enhancing the chances of fertilisation. In addition, ovulation induction tablets such as ‘clomid’ may be given which can cause the release of two or more eggs, increasing the chance of pregnancy (and multiple pregnancy, 6-8%) further.
Why Consider IUI After Tubal Reversal?
- Enhanced Fertility Chances: While tubal reversal restores the natural pathway for eggs and sperm, it doesn’t guarantee pregnancy. IUI can boost the chances by ensuring a higher concentration of sperm reaches the egg.
- Addressing Other Fertility Issues: Sometimes, other factors like mild male infertility or cervical mucus problems can hinder conception. IUI can help overcome these barriers.
- Less Invasive and Cost-Effective: Compared to In Vitro Fertilisation (IVF), IUI is less invasive and more affordable, making it an attractive option for many couples.
The IUI Process
- Ovulation Monitoring: The process begins with monitoring the woman’s ovulation cycle. This can be done through blood tests, ultrasounds, and ovulation predictor kits. With Dr Dobson, providing you have completed your pre treatment consultation, once your period starts you will contact Mandy to let her know you are ready to have IUI that month. You may also be advised to take an ovulation induction tablet such as Letrozole or Clomid from days 2 to 6. You will then arrange a scan on day 10 of your cycle either with Dr Dobson at the Park clinic, Nottingham, or alternatively at a local ultrasound clinic. Dr Dobson will then review this scan with you on the day and advise you when to start twice daily home LH urine testing. Once you get an LH surge you would then contact Mandy/ the IUI clinic (details would be provided) and travel up to Nottingham (just one trip) for the IUI 1-2 days after the LH surge. The actually IUI procedure takes place at the local IVF clinic/ lab Dr Dobson utilises near Junction 25 of the M1.
- Sperm Collection and Preparation: On the day of the IUI, a sperm sample is collected from the partner or a donor. The sperm is then washed and concentrated to increase the chances of successful fertilisation.
- Insemination: Using a thin catheter, the prepared sperm is placed directly into the uterus. The procedure is quick and usually painless, similar to a Pap/ cervical smear.
- Post-Procedure: After the insemination, the woman can resume normal activities. A pregnancy test is typically done two weeks later to check for success.
Success Rates and Considerations
The success rates of IUI after tubal reversal can vary based on several factors, including the woman’s age, the quality of the sperm, and the overall health of the reproductive system. On average, IUI has a success rate of about 10-20% per cycle. It’s important to have realistic expectations and discuss all options with a fertility specialist such as Dr Dobson.
Conclusion
IUI after tubal reversal offers a promising pathway for many women looking to conceive. By combining the benefits of restored natural fertility with the advantages of assisted reproductive technology, couples can increase their chances of achieving a successful pregnancy. Always consult with a healthcare provider to determine the best approach tailored to your unique situation.
